The SustematiQ website and the FoQus application are operated by the following service provider:

Electronic Services Disclosures (PSEM)

The Polish Act of 18 July 2002 on Providing Services by Electronic Means (Ustawa o świadczeniu usług drogą elektroniczną) requires us to give you this information: our identity and contact details are above (Article 5); the risks of using electronic services and the function and purpose of software or data that we place on your device are below (Article 6). The rules for our services, which Article 8 requires, are in our Terms of Use.

• Technical Requirements: Using the website and the FoQus app requires a device connected to the internet and a modern, up-to-date web browser (such as Chrome, Firefox, Safari, or Edge) with JavaScript enabled, supporting cookies and localStorage. Sending a form, signing in or creating an online FoQus profile also requires an email address.
• Service Risks: Utilizing electronic services carries typical internet-related security risks, such as unauthorized data access, malware, computer viruses, phishing, and service interruptions. Users are advised to take active protection measures, including maintaining up-to-date operating systems, browsers, and antivirus software.
• Software & Data Functions: Information about the functions and purposes of data or software introduced into the user's terminal equipment that are not part of the service content (such as cookies or localStorage) is available in our Cookie Policy.
